Payment Instructions
To make an online payment, please do the following:
- Locate your Account Number on the upper right hand corner of your Association Dues Statement;
- Visit the Curtis PayLease page and click "Login", or if you do not yet have a PayLease account click "Create Account" toward the bottom of the form; and
- Complete the PayLease forms for making an online payment.

Form Instructions
To sign up for automatic payments please do the following:
- Complete and sign the electronic Auto Debit Form below;
- When you receive the "Confirm your signature" email from Adobe Sign, please click "Confirm my email address";
- Simply complete the process again if your bank account requires multiple signatures.
